Membership
2021-2022 SEASON
1. Memberships are good for one year from the date of purchase.
2. Complete the membership application and submit the application to the club.
3. List all Tennis Club members on the application.
4. All members must read and sign the waiver. A parent/guardian must sign the waiver for a child 17 years or younger.
5. Submit the application to the Tennis & Health Club. A representative will contact you for payment.
